The Care Coordination program assesses the
needs of persons 60 years of age or older and, when appropriate, helps them
access and coordinate local resources to support independent living.
Priority is given to those who:

Have recently suffered a major illness or
health care crisis, or have recently been hospitalized and need additional
attention during the recuperation period;

Are moderately to severely impaired in
activities of daily living and instrumental activities of daily living;

Have insufficient caregiver support;
-
Are in greatest economic/social need,
particularly low income, minority seniors.
Direct services include:

Minor residential repair for eligible
clients when a health or safety issue is identified.

Minor health maintenance equipment not
provided through Medicare or Medicaid.

Limited homemaker assistance as determined
by assessment.

Limited personal assistance as determined
by assessment.

Limited one-time prescription drug
assistance.
-
Limited help with final-notice utility
bills to prevent having them disconnected.
