Chronic Disease Self-Management Program (CDSMP)

Chronic diseases such as heart disease, diabetes, and high blood pressure are widespread though out Bexar County.  My Health Matters will offer the Stanford University Chronic Disease Self-Management Program (CDSMP) in select Catholic Churches.  The goal of the program is to empower older persons with chronic diseases to manage their health and maintain active, fulfilling lives. 

About CDSMP

CDSMP is a six-week workshop offered once a week for two and a half hours.  People with different chronic health problems attend together.   CDSMP workshops are facilitated by two trained leaders, one or both of whom are non-health professionals with a chronic disease.

Program Topics

Subjects covered include:

  • Techniques to deal with problems such as frustration, fatigue, pain and isolation
  • Appropriate exercise for maintaining and improving strength, flexibility, and endurance
  • Appropriate use of medications
  • Communicating effectively with family, friends, and health professionals
  • Nutrition
  • How to evaluate new treatments.

Participants Receive:

  • Living a Healthy Life With Chronic Conditions, the program companion book
  • Time for Healing, an audio relaxation tape
