Chronic Disease Self-Management Program (CDSMP)

Chronic
diseases such as heart disease, diabetes, and high blood pressure are widespread
though out Bexar County. My Health Matters offers the Stanford University
Chronic Disease Self-Management Program (CDSMP) in select locations. The
goal of the program is to empower older persons with on-going health concerns to
manage their health and maintain active, fulfilling lives.
About CDSMP
CDSMP is a seven-week workshop offered once a
week for two and a half hours. People with different on-going health problems
attend together. The workshops are facilitated by two trained leaders, one or
both of whom are non-health professionals with a chronic condition.
Program Topics
Subjects covered include:

Techniques to
deal with problems such as frustration, fatigue, pain and isolation

Appropriate
exercise for maintaining and improving strength, flexibility, and endurance

Appropriate use
of medications
-
Communicating
effectively with family, friends, and health professionals

Nutrition
-
How to evaluate
new treatments.
